Take the confidential Personal Health Assessment (PHA) and receive a chance to win one of eight $25 gift certificates.

It’s a great opportunity to find out your current health status and how your lifestyle behaviors today can affect your health in the future.

What do I get?

  • A customized Health Assessment Report.
  • An invitation, when you qualify, to be enrolled in lifestyle management programs designed to give you one-on-one attention and tools to help you make healthy behavior changes.

Personal Health Assessment Instructions

The confidential Personal Health Assessment (PHA) is a tool that assesses your health status and provides you with a customized health report.  Blue Cross Blue Shield of Massachusetts uses the information you provide in the PHA and the results to identify programs that might be applicable and useful to you. This information is not shared with your employer.

Use the following steps to complete your PHA now.

Simply:

 

  • Click on Member Self Service
  • Click on Register Now. If you are not a registered user, create a username and password

 

  • Once registered, click on the MyBlueHealth icon and link for the Personal Health Assessment
  • Complete the PHA and receive a chance to win one of eight $25 gift certificates (when you opt to share your information with Blue Cross Blue Shield of Massachusetts)

 

  • Review your results
