Medical Forms

Physicals: According to Massachusetts state law, physical examinations are required upon entry to the school system (kindergarten and transferring students) and every three to four years thereafter. Download the Physician's Report to School.

Certificate of Immunization: This form is required befoe entering school. Download the Massachusetts Department of Public Health Certificate of Immunization.

Sports

An annual physical is required for participation in school sponsored athletic activities. Special sports physical forms are available in the health offices as well as in the athletic department. It is preferable that students have the physical performed by their primary care provider. All physicals must be current to participate. Download the Sports Physical form.
